What we provide
We have skilled clinicians who can carry out Quality visits for children and young people and adults. This includes quality visits for people that are placed outside of the local area to mental health hospitals. This may include where there are quality concerns about a mental health hospital including services for people with learning disabilities and autistic people. There may need to be a review of a person experiencing restrictive practices in hospital or the person may be subject to long term segregation or seclusion and need support on an exit plan or reintegration. We can complete 8 weekly quality visits for all people currently placed out of area or provide an independent quality visit where there maybe concerns.
Core aims
· Complete oversight visits (8 weekly or as agreed)
Ensure quality of care and education and treatment being received in hospital is good.
· Identify barriers to repatriation/discharge
· Work with existing clinical teams to ensure collaboration and discharge goals
· Act as a bridge between home and host ICBs
· End goal: repatriate people out of area, or find safe local homes/support in the host area where appropriate
Common challenges we help unblock
· Some people are not clinically ready for discharge, or have MoJ restrictions
· Resource-heavy coordination across multiple organisations
· Limited knowledge/relationships between home and local teams/services
· Difficulties accessing local services when someone stays in the host area
· Long-distance transitions where a slow, supported transition is needed
· New referrals discovered late, or transfers out of area by provider trusts
How oversight visits work
· Allocate/planning trips efficiently where geographically possible
· Build relationships with wards, local authority and community providers
· Structured visit framework: safety, rights, restrictions, progress, discharge planning, family voice
· Feedback loop to families, commissioners and providers with actions and escalation

· Oversight visit summaries
· Barrier-to-discharge tracker with solutions and accountable owners
· Risk and safeguarding escalation notes (as required)
· Commissioner-ready progress reports
