We deliver C(E)TRs in person, with careful preparation beforehand. Strong preparation is what turns a meeting into real progress: the right people are invited, the right information is gathered, key risks and questions are clarified, and the person and family are supported to contribute meaningfully. This increases the quality of discussion, reduces repeat meetings, and results in clearer recommendations, actions and timescales.

Complex discharges often stall because ownership is unclear, decisions are delayed, or planning is fragmented across health, social care and providers. We add practical capacity and coordination—building one plan with clear actions, owners and deadlines, and escalating blocks quickly through agreed routes. Our focus is progress with safety: aligning risk management, provider options, transitions, funding pathways and community readiness so the plan moves forward without creating avoidable risk or instability.

We provide reporting that commissioners and providers can actually use. You’ll see progress against agreed actions, the barriers preventing movement, current risks (including safeguarding concerns where relevant), and the next decisions required—plus who needs to make them.
