What we provide

Case management for individuals with mental health, learning disabilities and autistic people who are in mental health hospitals without clear discharge plans. This includes people who have increased lengths of stay in hospital, or experience repeat admissions to hospital. This will include a collaborative approach from our case managers, planning and facilitating and advocating for options and services that can meet an individual needs in the community. We have a wealth of knowledge around complex cases and commissioned care which is personalized to meeting the needs of individuals to enable discharge. This may include attendance at MDT ward rounds and planning meetings, CPA meetings, and CTR/CETR meetings. The approach will be multiagency and collaborative and will have clear and concise action plans to support effective discharge.

What we do (practical case management)

· Create a single, clear discharge plan with owned actions and timescales

· Coordinate meetings with the right decision-makers present

· Align health, social care, commissioning, and providers around one plan and centred around the individuals wants and needs.

· Support provider search/matching and transition planning

· Support with developing risk management and contingency plans to reduce placement breakdown

· Escalate decision blocks quickly through agreed governance routes
